ICSI Safe Site Protocol
The Institute for Clinical Systems
Improvement ( ICSI )
has released its new Safe Site Protocol for All Invasive High-
Risk or Surgical Procedures (Protocol).
The New York State Department of
Health (NYSDOH) released its revised New York State Surgical and Invasive
Procedure Protocol (NYSSIPP) in late 2006.
Both emphasize the importance of
repeating the timeout verification process whenever there is a change in
personnel involved or a change in location of the patient.
Both closely mirror Joint Commission’s Universal Protocol for Preventing Wrong Site, Wrong
Procedure, and
Wrong Person Surgery.

Correct Site Surgery Initiative. Not only do they have a brochure for patients but they also used a tool from IHI to do a
Failure Mode and Effects Analysis for use of an OR
Verification List.
